I here by declare that I am a citizen of India. All the particulars stated in this application
are true to the best of my knowledge and belief. I have read the Information Brochure and the
Admission procedure and I shall abide by the terms and conditions contained there in. In the
event of suppression or distortion on any fact like Qualifications, Category etc. mentioned in
my application form, I understand that I will be denied the opportunity to appear in the UKPGMEE-2016
and if already admitted, my admission is liable for Cancellation. I also
understand that the decision of the authorities of the UKPGMEE-2016 regarding
my admission will be final and shall abide by their descision. Further, if admitted, I promise to
abide by the rules and regulations of the institute as well as UKPGMEE-2016.
